Xigduo XR is type-2 diabetes medication that combines metformin HCI with the SGLT2 inhibitor. The combination therapy was FDA-approved to help diabetic patients improve glycemic control.
However, the federal agency has since warned that sodium-glucose cotransporter-2 (SGLT2) inhibitors have caused multiple cases of diabetic ketoacidosis, ketoacidosis, or ketosis.
According to the U.S. Food and Drug Administration, between March 2013 and June 2014 type-2 diabetes patients reported that SGLT2 inhibitors caused 20 cases of ketoacidosis, ketoacidosis, or ketosis. The agency further states it received more adverse event reports regarding a link between SGLT2 inhibitors and ketoacidosis since June 2014.
Ketoacidosis is a serious medical condition in which the body produces high levels of ketones (blood acids). Ketones can accumulate in the body when insulin levels are too low or during periods of prolonged fasting.
The FDA warns patients taking SGLT2 inhibitors should watch for signs of ketoacidosis and get medical help immediately if they notice any of the following symptoms:
- Unusual fatigue and sleepiness
- Abdominal pain
- Nausea
- Confusion
- Vomiting
- Difficulty breathing
According to the federal agency, physicians should confirm whether a patient shows signs of ketoacidosis and encourage their patient to discontinue taking an SGLT2 inhibitor if acidosis is confirmed.
The FDA reports that other possible side effects of SGLT2 inhibitors include kidney complications, dehydration and low blood sugar. When combined with other medications, SGLT2 inhibitor side effects have been known to increase cholesterol levels and cause yeast infections.
What Are SGLT2 Inhibitors?
Sodium-glucose cotransporter-2 inhibitors are a class of drugs that effectively treats type-2 diabetes by causing the kidneys to remove sugar from the body and secreting it into the urine. Manufacturers claim that when combined with diet and exercise, SGLT2 inhibitors are an efficient treatment of type-2 diabetes.

- Jardiance (empagliflozin)
- Invokana (canagliflozin)
- Farxiga (dapagliflozin)
