Diabetes Drug Invokana Linked to Kidney Failure

kidney diseaseDiabetes patients have several options for medication these days, and many of those options are new to the market. One of the newer diabetes medications, Invokana, may come with the risk of developing kidney failure.

Invokana (canagliflozin) was first approved by the Food and Drug Administration (FDA) in 2013, is indicated for use in conjunction with diet and exercise to keep glycemic levels under control in adult patients with type-2 diabetes.

Invokana is one of a class of diabetes medications known as sodium glucose co-transporter 2 inhibitors, or SGLT2 inhibitors. These medications work by blocking the action of an enzyme that causes reabsorption of sugar into the blood through the kidneys.

SGLT2 inhibitors such as Invokana prevent that reabsorption. The excess sugar is excreted in the urine, and the level of blood sugar remains lower than it would be otherwise. Some clinical studies showed Invokana patients experienced a small amount of weight loss, which could be attributed to the way the drug blocks reabsorption of glucose.

Invokana and Kidney Failure

However, other Invokana side effects, such as urinary tract infections and kidney problems, have not been so beneficial to diabetes patient.

The warning information for Invokana shows that some clinical trial subjects came down with infections in the kidneys or elsewhere in the urinary tract. Kidney problems are listed among Invokana side effects, and patients with existing kidney problems may not be able to take Invokana safely.

In a December 2015 drug safety communication, the FDA alerted the public to the possibility of kidney failure associated with SGLT2 inhibitors such as Invokana. The federal agency reported that it had identified 19 cases of life-threatening blood and kidney infections in its adverse event reporting system. All 19 subjects required hospitalization, and some required intensive care treatment for kidney failure.

With that announcement, the FDA also mandated a change in the labeling for Invokana and all other SGLT2 inhibitors to reflect reports of kidney problems and other side effects. The federal agency also ordered drug manufacturers to conduct a postmarket study relevant to those reports.

Despite the possibility of kidney failure, sales of Invokana have brought ample revenue to Janssen and its parent company Johnson & Johnson. Invokana is now the top-selling SGLT2 inhibitor on the market, according to Janssen. Sales have increased quickly in the short time the drug has been on the market.

While the prospect of losing weight on Invokana may have contributed to the drug’s outstanding sales, J&J has still invested heavily in marketing to promote the drug. One report from ProPublica shows J&J spent millions promoting Invokana directly to physicians, including millions spent on speaker’s fees, consulting fees, and food and drinks.

Invokana Lawsuits

Patients suffering from alleged Invokana side effects have only recently begun to file Invokana lawsuits. Generally, these plaintiffs are alleging that Janssen and J&J failed to properly warn them about the risks of kidney failure and other complications associated with Invokana. Had they been properly informed, they say, they could have avoided developing those side effects by choosing to take a different diabetes medication.
