FDA Warns of Ketoacidosis Risk with Diabetes Medications

SGLT2 inhibitors are approved by the FDA for use with diet and exercise to lower blood sugar in adult patients with type-2 diabetes. These diabetes medications work by causing the kidneys to remove sugar from the body by secreting it in the urine.

The FDA’s Adverse Event Reporting System (FAERS) indicated 20 cases of diabetic ketoacidosis, ketoacidosis or ketosis in patients treated with SGLT2 inhibitors between March 2013 and June 6, 2014. These patients required hospitalization or visits to the emergency room to treat the ketoacidosis. The agency continues to receive reports of diabetic ketoacidosis and ketoacidosis in patients treated with the SGLT2 inhibitors.

Acidosis occurs when there is an increased acidity in the body fluid. Ketoacidosis refers to a serious condition in which the body produces high levels of ketones (blood acids). Ketones can accumulate in the body when insulin levels are too low or during periods of prolonged fasting.

In a Safety Announcement issued on May 15, 2015, the FDA warned patients taking SGLT2 inhibitors to watch out for symptoms of ketoacidosis, such as:

  • Difficulty breathing
  • Abdominal pain
  • Nausea
  • Vomiting
  • Confusion
  • Unusual levels of fatigue or sleepiness
